Healthy Active Workplaces Online Employee Health and Wellbeing Survey
The resource kit for physical activity and health in the workplace highlights the need for workplace health and wellbeing programs to be based on the needs of their staff.
To help your organisation identify these needs and issues, the Department of Sport and Recreation and the Physical Activity Taskforce have developed an online employee survey tool that:
- Provides your organisation with an already established workplace health and wellbeing survey that they can distribute to staff electronically (or in hardcopy). The survey includes questions relating to smoking, nutrition, physical activity, stress and alcohol consumption.
- Provides individual staff who complete the voluntary survey with immediate confidential feedback on their health and wellbeing.
- Provides your organisation with a summary of your employee health and wellbeing findings which can assist you to design an effective workplace program (please note that this report does not provide information on individual employees).
Organisations
Step 1 - Register your organisation and set up your survey
Register your organisation to participate in the Healthy Active Workplaces Online Employee Health and Wellbeing Survey.
Once you have registered your organisation you can individualise your Healthy Active Workplaces Employee Online Survey . You will need to provide your contact details and identify whether you would like to include questions about your organisation's location and/or job types.
Step 2 - Distribute your survey
Once you have saved your Healthy Active Workplaces Employee Online Survey you can generate an email invitation for distributing your online survey to your employees or alternatively you can print hardcopies of your survey for distribution.
Step 3 - Check the progress of your survey
Once you have distributed your survey to your employees you can monitor the response rate for your survey, choose when your survey will close and view your organisational report.
Individuals
Individuals can also complete a Healthy Active Workplaces Online Survey to assess their health and wellbeing and receive individual feedback. Please note that the survey has been designed for use by an employee of an organisation and so some questions may not be relevant.
